For any referrals, the following documents must be submitted to Benchmark Valley 24 hours prior to admission per AHCCCS rules:

  1. Face sheet showing insurance plan and diagnosis
  2. History and physical
  3. Psychiatric evaluation
  4. Medication list
  5. Negative TB test and COVID-19 screening – no packet will be reviewed without a TB test
  6. Court order paperwork and probation officer contacts (if applicable)
